2012 LICENSE & REGISTRATION FORMS

• Competition License Application / Renewal
(This form is to be used if applying by mail or fax only. Applications submitted at the track use forms available there.)

• Associate Member Application
Associate Members are not allowed to race but may be the Owner or Captain of an Endurance team. Associate Membership is open to anyone who wants to support and participate as a non-racer with the CMRA. Associate Members get full access to post on the CMRA Message Board.
Fees paid by Associate Members wishing to upgrade during the year will be applied toward the Full Membership (competition license) fees.

• CMRA Annual Minor Release Form
(This form is required if license applicant is under 18 years of age. THE MINOR AND BOTH PARENTS OR ALL GUARDIANS OF THE MINOR MUST SIGN THIS FORM. If faxing or mailing this form, all pages must be signed/notarized.)

•Endurance Team Registration Form
(Team must be registered using this form before a pre-entry can be accepted) Team Owners and Captains must be a member of the CMRA (hold a current race license or a current Associate Membership).

Must be received in the CMRA office no later than midnight the Friday one week prior to the event. (8 days prior to the opening of practice)

CLASS SPONSORS AND CONTINGENCY

You must have the CLASS SPONSOR OF RACES YOU ENTER and CONTINGENCY PROVIDERS FOR EQUIPMENT YOU USE on your Sponsor List or you will NOT be paid the award offered by the Class Sponsor or contingency provider. Use the Sponsor List Form linked below and turn it in at registration (or with your Pre-Entry to the CMRA Office). SPONSOR LISTS CAN NOT BE CHANGED AFTER REGISTRATION CLOSES AT EACH RACE. Check the results page promptly after each race and contact the CMRA Office immediately if you think we made an error on your sponsor list.
